Momentum at South Bay for Rehabilitation and Nursing

This is my first time staying at a facility such as this, and I have to say my experience has been very good. The entire staff have been very attentive, very aware of my condition and have applied every technique they have from nursing, to OT and PT, to assist me in reaching a point where I am now able to go home. They are very pleasant people and I feel like they’ve become my friends. I will miss them. Also the recreation department has been absolutely incredible. . Make sure there are plenty of activities for all the residents and keep things going and run a very good program. I have told all my family members if they know that lives in the area the surrounding areas needs any OT or PT this is the place to come. It’s an excellent facility with wonderful people. So, a very unpleasant and stressful experience for me was made a lot easier because of Momentum and staffing. In addition, I neglected to mention the housekeeping staff who ensure that the room is clean daily, which is good for someone with my breathing condition.

I had an excellent experience. Rehab was terrific. Eric, Colleen and student nick were fantastic. They worked as a team. Nursing cna Guzman was outstanding. Room was spotless and comfortable. Outdoor patio was serenity. Food was ok but you had a special menu to chooses preferences from. Recreation Juliana was above and beyond. Caring and helpful. I would recommend momentum to others. Mary the psychologist was just what the dr ordered.

Your facility was inviting and the bakery area is a nice place to enjoy coffee and visit. I felt like we were out together at a quaint lil cafe. Please keep up the good work. I would definitely refer loved ones to your Facility!❤

Care is the most important thing when you become a patient. I have been at Momentum 3x and I love it here. Therapy dept is great. Tony and Paul are excellent, Nursing is wonderful. Carla the CNA is great. Food is good. If you dont like something you ask for something else. Recreation is great. All the events are well planned and great. You have to take advantage of whats good and look at all they have to offer. Momentum offers everything. I would recommend Momentum to anyone looking for a rehab facility. I can not find a complaint to add.

Momentum has recently provided outstanding care to both of my parents. My mom had to be admitted twice because of injuries related to falls and my dad was admitted with her recently due to general weakness and deconditioning related to cancer treatment. The physical, Occupational and Speech therapy staff are completely devoted to every resident, so that they can gain back their independence and make as much progress as possible. Other support staff from the people at the front desk, to the nurses, clerical staff, social workers, nursing assistants, supervisors, mental health, and medical professionals were all equally top-notch as they addressed any problems that my parents developed during their stays so that they could get past them and continue to benefit from their therapies. I can’t say enough about the direct care and support staff and how much I appreciate what they have done for my parents and my family, as they were equally responsive to my concerns regarding each of my parents. I am presently working on a formal letter to the administration with specific names of people who touched my parents lives in such a positive way because I feel I must give them all the credit they deserve for their strong positive work ethic and devotion to helping my parents and the other patients at Momentum at South Bay.

This is my 2nd time my family member required Rehabilitation after hospitalization. There are not enough words to express how grateful I am that my family member is able to be given the phenomenal care at Momentum. The staff here go above and beyond to provide care. The facility itself is beautiful and immaculate.

Our family had a wonderful experience at Momentum. My Dad progressed tremendously after his TBI and he is now home with us. We are so grateful to the entire staff and personnel who made it possible. Thank you so much!!!

My experience at Momentum was terrific. When I first arrived I could not walk and my speech was off. My improvement was fantastic. Allison my Speech therapist was amazing, Tony and Sue my Physical Therapists were beyond helpful. The staff are very motivating and really care. Therapy was A+. They push you and motivate you to keep you going. Nursing staff were professional and go above and beyond. Andrea and Asia were fantastic. Food was good B+. Bakery was A+. Recreation had fabulous offers for programs and covered all areas of interest. Although I did not participate in much, I was amazed at all the activities offered. There was enough to keep you busy all day long. Even if you don't attend, staff were very inviting and social. I would recommend Momentum to everyone.

Check in was tonight, it went very smooth.
Facility so far is clean and the staff is welcoming and friendly. I know there’s procedures and Pop will be here for a long while, so we will update as patient settles in.

Over the past few months of being here I have always felt that the nurses were very supportive and attentive (especially Brianna, Kelly, and Dawn). Additionally, the nurses aides (Samilia, Marie, Kevin & Judy) and the PT/OT staff (Christine and Colleen) were excellent! I would definitely recommend this place to people who need rehabilitation!

Update: My mom is back and she is enjoying all the activities Momentum has to offer. Pet therapy, art, wonderful entertainment, games, a beautiful patio to sit in, and this week has been a ball. Nursing home week-Great theme, great food and entertainment. She is having a ball. Recreation staff are so attentive and encouraging. I truly appreciate all staff at Momentum by making a difficult time more manageable.

My mother has been in and out of different nursing facilities in Suffolk County for the past 3 years. I have to say Momentum has been the best one. My mom received wonderful care, excellent rehab team,, they listen to your concerns and they care. I have not had many good experiences with several other facilities. Especially during the pandemic, Momentum does their best to accommodate families and my mom was very happy there. I would recommend this facility to my family and friends.

Early impressions great. Very clean, private room. Therapy department is very impressive. Responsive to concerns. Communication excellent.

Great facility staff was so nice and took care of my husband Mr Kulla very well the service was great and never had any complaints I never had to worry about my husband while he was there

Having been a patient here at Momentum Rehab for almost 3 months now. I think I have been able to get a keen perspective into the facility. I shattered my leg and had to undergo a few surgeries, recuperate, then go for another surgery. The Wound Care team has tended to my wounds, even my grafts, with the greatest concern. He has worked patiently with me as I healed many times in excruciating pain. The Physical Therapy and Occupational Therapy I have received here has been beyond what I expected. I have had the same two people assigned to me, which I find awesome. There was a perfect balance of patience and pressure to push myself. With every setback or progress, they adjusted my workout to move me forward. In all honesty, I wish I could take them home with me, as I still have a long road ahead, but I can’t so instead they worked very diligently with me to help achieve the goals I need in order to go home with confidence.

I love the fact that I have the same nurses and aides. It has allowed me the care that I need without having to explain my unique situation over and over again. They began to feel like my guardian angels. They felt empathy when I cried in pain, when I cried in frustration, and never hesitated to contact the doctor when I felt something wasn’t right. They certainly were in here working out of love, love for people, and you could feel it.

It seems that even the people who might be overlooked, like the cleaning staff ( who come in with smiles not only ready to clean my room but go the extra mile to make sure I’m happy), the kitchen staff ( who never complained when I wanted to adjust any of my meals, which was usually everyday ) or those who work in concierge ( who must have super energy because they make a million trips to peoples rooms making deliveries or checking in on us) put their heart and soul into making sure the patients are happy and well taken care of. You definitely develop a wonderful relationship with those who work with you every day. My injury has caused me to feel depressed. They even have an awesome psychologist that has helped me through my negative thoughts. She seems to pop in just when I need her and doesn’t get insulted if I don’t. I would imagine she is one of the gifts this facility has that many may not be aware of.

Now, being here as long as I have, I think I have also seen some faults. The nursing staff along with the aides seem to have suddenly become overworked. In my opinion, I think with an influx of patients they have become understaffed. This has led to Great Nurses and Great Aides not being able to give equal attention to their patients. Now with visitors, arranging that privilege along with the Covid restrictions, has pulled people from their normal jobs, like in the concierge department, to now having to oversee the visitors outside. I feel if more staff can be hired and trained for these new situations, it will be the facility I fell in love with.

I will say though, that anytime I have complained, because of course when you’re in a facility for three months you’re going to find something to complain about, in Momentum's defense, they have always tried and usually succeeded in making me, the patient, happy.

🦋 I am editing this, as I am now leaving in 4 days. It’s been 3 months 10 days. I thought I covered everything, I didn’t.
The Social Workers have set up everything I need to go home, and I need a lot. They have dealt patiently with me as my anxiety of going back into the real world with a full leg cast, non weight bearing, plus my other health issues that I had, ( I am on SSDI ) prior to coming here. They listened to my concerns and have worked with my insurance, filling out the medically necessity forms, so that my transition home and my remaining healing will be able to be done safely.

I am changing my rating from a 4 to a 5 because Momentum and its staff has gotten me through one of the worst parts of my life. I will forever be grateful.

Very happy with the care here. The facility is clean and everyone is helpful and friendly. Would definitely recommend.

Love this place for my family members. First nursing facility that I have seen fully staffed most of the time. They always have plenty of supplies if my family needs, plenty of towels, diapers, sheets, etc. It’s also not often you see a bakery with a pizzeria in a nursing facility. They have so much to offer and PT is excellent and therapist always willing to assist with just about anything you need.

The only negative thing, isn't really a major issue it's your 1st time visiting & the time it takes to register. Very secure, but lengthy. Otherwise, my sister is in a nice clean place where the aides seem caring & pleasant. The nursing station is centralized & easy to get to.

I had a very positive experience at Momentum rehab. I chose Momentum after my family searched for a facility with positive reviews and since prior facilities, I had experience with, I wanted to stay away from. The best part of Momentum was the professionalism of the staff. The admissions process was smooth, accommodations were satisfactory, food service was good, nice variety of food. nursing care was great. Nurse Kelly was above and beyond supportive. Client services was responsive to all my needs. MD were attentive, social work was involved and supportive in my discharge, Rehab Kelly and Steve were excellent! Recreation staff was very friendly, encouraging and great people. I enjoyed all the activities I took part in. I met some really nice people there. My stay was overall a positive experience, and I would go back If I had to. I would recommend them to everyone I know that needs rehab.

Where do I start? I never expected to need a rehab facility, but I thoroughly enjoyed (I’m not exaggerating) my stay at Momentum. The facility is clean and all of the different staff members are professionals. They deal with a diverse patient population that would, in all likelihood, drive me nuts, but they come through with flying colors. The nursing staff that handled my wound care, Nicole (days) and David (nights) were FANATASTIC. The aids, Michelle and Bianca in particular, were also terrific considering all of the different duties they had to perform.

The therapy staff, both physical (Kelly) and Occupational (Christine) were thorough, knowledgeable of the body, and never ran out of exercises for me or any of the patients to do. Thank you Therapy!!!

Now we get to the recreation crew. These people are incredible. There are many recreational activities every day and the crew is in perpetual motion. They not only go to patients’ rooms to remind them of activities, but they also transport them to the events. We had so many events including music, bingo, outdoor barbeques, and best of all, my favorite, card games. Kathy, Jen, Julianna, Silvia, Brianna and Sam are all very caring individuals who make you feel as though you are the most important person in the world. How the entire staff remembers everyone’s name is incredible.

I also have to mention the bakery and their staff. They are so accommodating and friendly, and have delicious baked treats (both with and without sugar) and even pizza! Thank you to everyone for making my stay so comfortable and enjoyable!

I was personally in Momentum from March 23 -May 23, several of the staff were excellent. The food was horrible, thank goodness for Uber Eats. My aides and RN’S am/pm were great, except 1. They were very warm and attentive also welcoming the entire time I was there. PT/OT were great. I had 1 problem spoke to my wound care RN Khalid who took care of her not being my therapist anymore. Khalid went above and beyond to get me a referral to Dr Adler plastic surgeon in Mather Hosp to have my surgery done in a timely order. I would recommend Momentum for anyone who is able. Thank you all from Medical to Administration staff I was treated fairly.
