My mother was here for over 2 months. The staff is super friendly, patient and care. The on site activities they held during the holidays were a big hit. The facilities are well kept and the place is really clean. The staff will go out of their way to help you anyway they can. This was a stressful time for me and I can't thank them enough. A special thank you to Shannon for helping me get what I needed taken care of for my mom.

My Aunt is a resident of Pulaski Health and Rehab. The staff there are wonderful to her from the Administrator to Maintenance. All staff, residents and families are just like a big family. That’s what makes facility so wonderful. The RN’s and Aides are very compassionate and caring for the residents.

This facility has very caring staff and took time with me and was always patient. I am legally blind and they were very supportive and patient in that respect. The therapy was excellent and knowledgeable staff. They got my strength built back up where I am able to go home . I would recommend this facility for others who may need it !

I just wanted to say that I appreciate the Housekeeping and Laundry Department here. Everyone who has come into my room to clean up has been awesome and I’ve enjoyed how friendly they are. Barbara, you have a great group of people who work for you ❤

PHR had been wonderful to my mom. Very attentive to her needs and the rehab gym is great for all level of needs. I would recommend their facility to those who need post hospital assistance.

The building is very clean. The staff is amazing and they take such good care of my dad. Everyone there is so nice. They try so hard to meet my dad's needs.

Pulaski Health and Rehabilitation Center is the best place for me to work. I retired in 2019 after 14 years as a CNA. Now I am excited to be returning as a PCA. Retirement is just not for me. This place is very important to me as are the residents I will be talking to. I look forward to working with the team again.

Perhaps if I had known more about this center prior to my employment with Pulaski Health and Rehab, this is where I feel my mother would have succeeded in returning to her prior level of functioning. Lifeworks Rehab is where it is at! A state of the art gym, PT and OT at its best, nurses and CNA’s who provide personal and the best care, housekeepers who do their best, maintenance is there to fix any issues, social workers/discharge planners who actually help you get ready for your loved one’s transition home, and an administrator who thrives on the success of our patients and the staff! I am blessed to be apart of such a great team and organization!

My sister shattered her tibia in January. She insisted on going to a different facility and had a terrible experience. We then moved her to Pulaski where she is making great progress with their phenomenal rehab staff.

Each time I enter Pulaski Health and Rehab, throughout the building I am greeted with smiles and everyone is friendly. The residents look happy and the building is clean. The staff is knowledgeable and caring.

Went to visit my mother. The place is very clean. The staff is very helpful. They are taking such good care of my mom, she decided she really liked it there!! Impressed! She is hard to please.

I have only been a nurse for over a year but I feel like I have already found my "home" and see myself staying with this company for the foreseeable future. The staff cares for their patients and we all work together as a team. Our therapy department is amazing. I would recommend PHRC to anyone who needs rehab or even LTC.

Very professional and courtious staff. The facilities are very nice and clean. The nursing staff has taken very good care of my Grand father. Overall a great place.

As an employee here I see our team working hard every day to make our facility feel like home . Our team is dedicated to resident care , our activity department and our employee morale committee have been scheduling events that are fun and include staff, residents and families . My mother in law went into the nursing home in fair health. Robert Foster and aide working with him did great with her just doing their jobs the right way. The nurse's were great with her and us the family at the end. My wife, I and family were with her thru the end and the whole staff that was there were great. Thanks to everyone for everything y'all did. Carilion Hospice big thanks to everyone involved for the speedy changes when you're loved one needed it so no suffering was endured. Thanks to everyone not mentioned above that was involved also.
