Encompass Health Rehabilitation Hospital of Toledo

I had a hip replacement with some unforeseen complications. I had not planned to go to a rehabilitation hospital, but the surgical center was able to transfer me here. I cannot thank everyone enough for the great care that I received! Everyone I met, regardless of their particular job, was so kind and so helpful! The doctors, nurses and nursing techs were there when I needed them. The Physical and Occupational Therapies were top-notch. Even the food was great, and the kitchen was more than accommodating if I wanted to substitute for something. I will have the 2nd hip replaced within the next few months. While I hope it can be done on an out-patient basis, if I need more than home care, I hope Encompass will have a room for me! A big thanks to all! Deborah

The care my mom is receiving is PHENOMENAL. Thankful for all the staff but I'd like to spotlight Nurses Julie & Lisa. Case Manager Raquel, Aid Jam & the therapy team.

This was such a positive experience,I have shared it with several friends.
There were other options that had good reputations, but I wanted to stay close to home. I am so glad we decided on Encompass.
The case manager spoke with me on the first day and told me after OT, ST,and PT made their evaluations she would contact me with the plan and estimated discharge date. And she did! On therapy days, there was 3 hrs of therapy each day, as they promised. I was updated by each therapist and or the nurse about daily progress.
I went daily after work, everyone was very kind and respectful. When a new pt.is admitted, they announce a welcome over the intercom. One of the staff told me anyone that is available will go by and welcome them.
Patients are given a notebook with information for their progress, questions, recovery exercises, medication lists etc. The pharmacist put her phone number so you can call yourself with any questions.
While I was there over the 10 days, a respiratory therapist, and a pharmacist both answered his call light just because they were walking by. Everyone helps, even if they are not assigned to your loved one.They even arranged for transport to a f/u doctors appt. This was very helpful b/c I could not be there during the day.
The facility is beautiful, it is brand new, state of the art equipment . The building is secure, they will buz you in.
The discharge date was met, and by that time he was walking without any assist devices! I highly recommend Encompass in Toledo, and have shared that with all of his doctors.

This is my husband's 3rd visit. I don't want to take him to another place. Everyone is great, it's clean,therapy is awesome and foods not half bad. HIS MEDICAL needs are always ment. Best

The staff is very caring and friendly. The building is clean and comfortable and I would recommend the facility for anyone.

They are really helpful and work with you to get home. The staff is great and they keep inform about your progress and how you are doing. I would recommend anybody to this place for rehab.

Great place clean with wonderful staff

I can't say enough good things about this place the therapy staff is amazing they clean before and after you use any machines or other equipment they make you feel comfortable and do an amazing job everyone from the case managers to the aides and nurses and doctors are great the minute you need anything they help you right away I would recommend this place to anyone that needs to be inpatient for therapy

My sister is mentally handicapped , and the therapy was excellent! Concern was when she was discharged, as all her belongings were lost??? Victoria, the social worker go to, did not seem very concerned or helpful. Staff and cleanliness was very good.

Not even a one star for this facility, coming from a Master level nurse. Save your loved one from poor care and bed sores. Recently (10/23) my mother was admitted after a hospitalization. The facility is clean I will give them that. There was no family involvement in discharge planning. Calls to nurses, admissions, and case management were not returned. Never once did we meet the case manager or get a call back to discuss after care. Of course my mother wanted to return home, was it safe for her to do so? Absolutely not. Were we set up with needed assistive devices? No. Home care? No. Names of providers she needed to see that were consulted while on the facility? No. The day of discharge she was found to have a bedsore that was not there upon admit. She also fell multiple times after discharge and was in the er within 48 hours. Go somewhere else, do what I didn't and research reviews and care standards of facilities before putting your loved one here. I don't live in the area so unfortunately I failed to do this and it cost our family the future health and well-being of my mother.

I do not advise you choose this facility based solely on the character, demeanor and aggressive approach from the facilities Social Worker Victoria. Victoria lacks empathy and compassion and talked to my mother like a child during her stay there. As a social worker you’d think having traits such as tact and compassion would come natural, not in this case. My mother had a few incidents while at Encompass where in one case her oxygen had been unplugged for hours (leading EMS to her bedside) meals not being delivered (choices being other than what she’d selected) needing to be put into pajamas in the early afternoon as no coverage would be available for hours (due to lack of staff) and being discharged prematurely due to Victorias judgement (insurance overruled her better judgement). The experience has been a nightmare and unfortunately my mother after being discharged is back in ER the same evening. Do not take your loved to this facility.

Some of those black nurses not all to me have a attitude problem.when my mom was in there. I feel that they didn't care or gave her good care. mm

Do NOT take your family members here. We almost lost our dad because they refused to give him the medication he needed, and told him not to call HIS OWN doctor because they had a doctor in the facility. He’s just now coming out of a coma all because they don’t know what they’re doing. Not to mention the overnight nurses treated him poorly his entire stay. Absolutely no bedside manner. Even the nurses at the hospital told us not to send him back to encompass because he was denied his daily medication.

I am absolutely disgusted at the visitor policy for this facility. My aunt was recently transfered here from the hospital after a surgery for rehab.
Me and my Son (3y.o.) attempted to visit during normal visiting hours for her birthday and after requesting to find out which room she was in, we were told my son was not allowed to see her or be inside the facility. That we would only be allowed to see her through the window IF she was in her room.
They also refused to give her a card and balloon that we had brought for her.
Additionally she was not allowed to come outside to meet us either or at all for that matter.
My Aunt as well as my family entrusted this facility for her care and well being however they apparently have not taken into account the status of her condition and how much time her family may or may not have with her regardless of how temporary the stay at this facility may be.

So please be aware for anyone who has family with young children this would not be the facility to send your family members to.

The assistants with their patients hygiene is terrible. My father contracted a UTI while at their facility. They never washed my father nor showered him and he was there for about 17 days. Their service or care can be improved if they become patient conscious and not money conscious.
