I would like to take the time to acknowledge a employee whom provided exceptional care to my father during his physical therapy treatment. My father look forward to his treatment and is making great effort to recover from his stroke. He was moved from another facility which didn't care for the patient, but Mindi the physical therapist is very friendly and caring with my father and other clients. Mindi care enough to know her patients by their names and make them feel as if someone really cares about them . GREAT JOB MINDI

I have been discharged from this facility today. They have been kind and compassionate towards me . The Assistant Administrator Raj went up and beyond to make sure that I receive great care. The CNAs are very professional and I'm found of the friendships I have built in my short stay here at Jefferson

Treated me well

I worked there not everyone is like that I love my job and my residents my Mom was there too. They don't care because they are not trained you don't learn at no school sitting at a desk you roll up your selves and get on the floor I did it

WARNING!!! DON'T SEND YOUR LOVED ONE HERE!!! ZERO STARS......My sister was abused for a long time and the staff got away with it. This place is a coverup, the pictures they have on the website are a lie. The professionalism is not there, nurses don't care, and personal belongings are constantly stolen. Unsanitary treatment and urine blankets were placed over my sister's head while the staff laughed. She sat in urine diapers, her teeth rotten and hair matted. The staff constantly yelled and demeaned my sister to her face, as if she was not a human being. No physical therapy was offered for her care, and no proper grooming, bathing, or hygiene was given. My sister sat for hours waiting for food and water, the nurse on duty never helped her and said that it was not her job. The facility was receiving money for her care and nothing was done, nurses, doctors, and staff only worked there to receive a paycheck. This place should be inspected, investigated, and shut down. My loving sister passed suddenly in an uncaring environment and it was covered up. HORRIBLE, UNCOMPASSIONATE, LACK OF CARE, SHOULD BE SHUT DOWN IMMEDIATELY.

Where do I begin? I give this place NO STARS if I could!! This place is not welcoming but the admin staff will have you believing it is the best place on the planet. I should have known something was wrong when their own patient told me "RUN" or "ENTER AT YOUR OWN RISK". I decided to have my mom come here since it was only temporary for rehab and I must say my mom did not last 4 days in that facility. I went everyday because I had a nurse tell me that if I was not on top of the staff they would leave my mom there not do rehab work with her as a way of forcing her to stay LongTerm so they could collect money. I asked everyday about her vitals, I asked if in fact they checked them and I got an OF COURSE but NO ONE was ever able to provide me with numbers. That clearly was a LIE. When my mom was rushed to the hospital, her Glucose level was through the rough (over 700) and her BP lets not even talk about it. It was clear then that they did not do what they was supposed/ are PAID to do. These NURSES and LPN's unethical. My mom needed assistance with her food and they never assisted her and consistently stated that she was just out of it and did not want to participate. I would guess so she was almost in a COMATOSE STATE, thanks to you guys. When they sent my mom to the hospital, I was not even informed that my mom was sent there. THEY DID'NT EVEN CALL (SO MUCH FOR PROFESSIONALISM AND DOING YOUR JOB RIGHT?!) I had to call to call Jefferson Health Care (MORE LIKE NO CARE) to find out that my mom was in the hospital. This place is HORRIBLE. Not a place I would recommend not even to my worst enemy, hell I wouldn't place LUCIFER himself in there. My mom was always kept dirty, never had a clean diaper when I would get there. I do not know how this place has survived. Do not believe the hype, this place is all wrong. Those 4 and 5 star reviews are a LIE, probably staff wrote that. NO ONE IN HERE IS ANY GOOD. NOT EVEN THE CLEANING STAFF I could go on and on about this Horrible place and their Staff. Jefferson Healthcare should be SHUT DOWN!! As their very own patient said "ENTER AT YOUR OWN RISK". STAY AWAY!!

This is truly the worst place you can put someone. If you hate the person, then yes you should put them there! The staff are incompetent, uncaring and rude. It’s dirty dead cockroaches on the floor. Yes I have pictures. This place was recommended to me by the social worker at Ochsner hospital. I don’t know what is happening but healthcare workers nowadays are the worst ever I was a nurse for 20 years and I have never seen such blatant disregard for human beings. Administration is not better how this place is even open is beyond me no communication and they do steal things from the patient, I don’t understand how close can disappear if you’re not even responsible for washing it? My mom has “lost” several items and phone chargers. This place should be shut down or at the very least investigated!!!!!!

This is the worse place in the world to have your love one. How do clothes come up missing that you all do not have to wash. My aunt currently need a new bed and I have asked for 1 year or longer that when she see her PCP that she get a power chair so she can get out of her room. They lie about when they bath their residence until my sister and I had to stay on them about it. Some of the staff threaten the residence. I had to complain about that, and nothing was done at least not by them. A good woman to woman talk will get ppl together when needed. To top it off the state does nothing about it after making several complaints. This is so sad on all levels. The place need to be closed down asap. The management team is trash. If the top stinks then the whole facility will stink. Sorry the good have to suffer for the bad. They have some workers that care but not very many. Louisiana if you care about ppl. Please close this place down.

I’ve worked at this facility some time ago, and I’m sad and disheartened that it’s still in operation. I’ve worked overnight where I was assigned more than 40 patients, which is already very hard to manage. On top of that, there wasn’t enough supplies and linens for each patient, so there were many times patients were left soiled. The hallways and patient room floors were sticky and dirty and there was definitely and unpleasant smell in the facility. Worse of all, there was a rat infestation (which I thought was just a rumor at the time), until I saw one scurry down the hall into a patient’s room one night. I quit after 2 weeks. I say all of this to say: Please don’t send your loved here.

My husband has to lay in wet diaper for hours even when he will ask nurse to have CNA change When they did come in they off sheet and left him with out a mattress sheet when asked for his blanket reply was I don’t work in laundry. They are rude and insensitive and incompetent

These people be quick to say you quick your job, but never put the reason you quick. Almost every night I got to work I was on the hall by myself with about a 100 patients or more, there is no way one CNA and one nurse can cover all those people without most not getting proper care. The state really need to go through this nursing home at night. . They don’t deserve one star, the head people just want a body in the building, but I had to leave there and go home knowing g all these people could not get taking care of properly. I wish they all had the heart I have. The DON was put on the schedule a couple of times to work and she never showed I so that left one nurse working all 4 halls by herself with almost a 100 residents on each hall. This place is a disgrace, I wouldn’t put my dog in here.

My mom was there last year and received horrible care. Her conditions worsened there and had to get treated even further. Would never recommend anyone send their relatives or anyone at all there. Overbooked and understaffed as well.

If I could leave zero stars, I would. I was an employee for less than three months at this facility around 2016. I would NEVER allow a family member be placed in this facility. While there, we were constantly understaffed; nurses often with 35 or more patients. CNA's were constantly understaffed, which lead to insufficient care of residents at this facility. The facility itself was unclean and unsanitary. Roaches were found in all areas of the facility; nursing station, restrooms, eating areas, patient rooms, etc. I strongly would recommend another facility for any family member seeking treatment.

After seeing the inhumane treatment my mother received at Jefferson Healthcare Center, I wrote to local and state elected officials to demand an investigation. The state-led investigation cited this facility for its unsafe, unsanitary, and unprofessional environment. I slept at my mother's bedside during her final days on earth and got to witness first-hand how the residents are treated when visiting hours are over. Callous and cruel. Please look elsewhere for better care for your loved ones.
