Flagler Health and Rehabilitation Center

I had a very pleasant stay at Flagler H & R. Which included all aspects -- the CNA's, Nurses, and particularly the therapy staff. Trish and Ulia were amazing. Very attentive to detail and the needed therapy. I depended on several of the CNA's, V, Diane, Christian and Jeanine. Nurses Amy, Beth and Kilshondra were very attentive to my pain and took care of me in a very professional and friendly manner. Additionally, the social services office with Devin and Cicely were so helpful they got me out of here on time and i would go back to Flagler H&R if I ever needed Rehab again.

My mom is well taken care of by the nurses,cnas,therapists, nutritionists, housekeeping and administration. Her room and bedding are kept clean. My questions are answered or I am directed to the person on staff that can answer my question. I have no complaints and more importantly my mother has no complaints!

Don't let the building fool you! I needed to get my mother-in-law in there in a hurry and was very unsure of the process since she was being admitted from home and not from a hospital. KiYaana in admitting was absolutely wonderful and patient with me. She took all of my phone calls, even on the weekends, and I called a lot! She paired my mother-in-law up with the perfect roommate to help her out of her depression. The facility was clean, the staff was attentive and knowledgeable, medication was given promptly, physical therapy was successful, the food was excellent, and they made sure there was lots of fun activities for everyone to take part in. I base my opinion on the month my mother-in-law was there. Thank you, FH&RC!!

We moved my mother to flagler rehabilitation last week from grand oaks. The staff is very friendly and helpful. Mom was a nurse, so she can be critical, but she is very positive about her experience so far. She especially likes the physical therapy. Grand oaks was an absolute nightmare.

I visited a friend during lunch. They all enjoyed my photos n stories of butterflies, passion flower vine n fruit, n magnolia tree. Lunch looked good.

At first glance, very institutional. Kinda reminded me of One Flew Over the Cuckoo's Nest. LOL Three stars for the building. But the staff is what makes this place amazing. If I were rating the staff alone I'd give them 7 stars so, it evens out.

The lady at the front desk where I checked in was very professional and knowledgeable.The staffs that I encountered was very professional. The resident was very clean and presentable. I am very happy my friend is in friendly and clean environment

I'm currently at facility for thyroid storm rehab, I was a vegetable. The PT department-Christy, Chad, Trish, Mauri, Melinda etc.,got me walking and moving about, the physical therapy department is well equipment with compassionate proffessionals. Taylor head management helped me out tremendously, even when my insurance ran out-youngest compassionate upper management boss, who always stops through on his rounds around facility. CN Kinley, Diane, and several others also. Overall- if your trying to walk and talk again, good food, great location. You get what you put in.

You're very caring and helpful you can talk to them that give you information they take good care of their patients from what I can see I go there every day to see my dad and he is getting better and Things Are slowly coming around and working out they do a good job

The staff is very friendly and professional. The facility is very clean. And they also keep the rooms very clean. I would like to give a special shout out to nurse Lee Lee for keeping my friend Hilary Araujo in good condition. She does a awesome job. Thanks...

Some excellent staff! Most outstanding was Pierre who was so gentle, patient, knowledgeable and caring. Brooke was an excellent nurse as was Tori. Don’t remember everybody’s name but all were good. Katherine, Director of Nursing, was excellent and so kind.

To God be all the Glory 🙏 stopped by to visit a church member and to pray 🙏 the staff was very polite and all the patient's seem very happy and cared for.🙌🙌

I have appreciated the therapists that have been working with my dad so much. He has been getting speech, physical and occupational therapy at flagler rehab and they have been so kind and personal with him and our family as a whole.

Great place to work! Caring staff that provides great care for their residents! Their activities department makes sure the residents are engaged and happy! They provide an excellent pulmonary program with a local pulmonologist and excellent wound care program. Excellent physician coverage in the facility and readily available Medical Director.

My Mom, Joanne is a patient there and is being very well taken care of.
One time I mentioned to one of the staff members that her friends suggested that we look after her.
I would told that it would be impossible, even if we could get 24 hour health care.

My wife was in the rehab department. The staff were extremely helpful and caring, including all the nurses, housekeeping, helpers, front office and social services. The food was great, my wife loved it.
The therapy team was amazing, including physical and occupational therapy.
I will recommend Flagler health and rehabilitation center above any rehab center.

Today 12/10/2022 the local 4H club stopped by to sing Christmas Carols. Reminded me when I did that as a child. Was so nice to hear the children sing.
I find the facility very clean and well maintained. The entire staff are very attentive to all it's patients. Pets are also welcome after providing vaccine documentation. I have observed the staff making frequent rounds checking on patient welfare.

Mary, Supervisor was so helpful.. I had questions about my son Fred Sherman’s PT.. she took me right to the PT room and then introduced me to Linda.. She answered all my questions.. was very pleasant and listened to my concerns. I was very pleased with this because his Goal is to be able to stand and pivot so he can get into their SUV again. Very good day!

The admission process for my father was excellent. Both the admissions director and the administrator were involved in the process and even gave me their cell numbers in case I had any questions. The LPN who checked him in was very thorough and patient. I got there at a little after 11 AM and they even arranged to be sure he had lunch. The staff is very kind and understanding. Thank you!!
